Adapter pattern: pipeline-service is the stable engine,
pipeline-ktv registers its step_types and handlers independently.

- 17 KTV step handlers (audio/video/demucs/lyrics/music/subtitle/video)
- Step type metadata with categories (media/llm)
- load_ktv_adapter() one-call integration
- Depends on pipeline_service>=3.1.0
