feat(team-communication): 团队沟通部署固化 + 菜单入口

- scripts/create_tables.py: pipeline_agent_questions 四列幂等迁移
- bin/init_data.py: 种子 sdlc_general 团队角色+问题类型+role_type 代码表
- wwwroot/index.ui: 菜单加团队角色/问题类型
